Consultation/lifestyle form*

An essential preliminary form, to be completed by the client before any treatment and to be talked through by the therapist with the client prior to treatment.  Includes guidance notes on usage.  Document provided in a PDF format.

Price: £10.95 + VAT (approx. €11.95/US$15.90) 

Eyelash tint disclaimer*

A crucial form for any business offering eyelash tinting, this form is for the client to sign if they request a treatment without taking a patch test.  This waives their right to pursue you if there is any bad reaction to the chemicals used in the normal course of the treatment.

Price:  £5.95 + VAT (approx. €6.50/US$8.65)
